If you find yourself in a position where you would like to file a
formal complaint with our office, you may obtain the complaint form from
our website at
Arizona Attorney General, Terry Goddard. On the upper right side of the homepage,
you will see a “Quick Links” drop down menu. Click on the down
arrow and scroll to consumer complaints. You may then click on "File a
Complaint Online," to file electronically or "Mail in Complaint Form,"
if you would like to print and mail the form. Unfortunately, an email
does not constitute a formal complaint with our office.
You may also contact our office and we will be happy to mail you a form

Upon receipt, a member of our staff will review your complaint and
contact you by mail, in approximately 2-3 weeks, regarding any action
that can be taken by our office. If it is something our office does not
have jurisdiction over, we will notify you of the correct agency to
contact and forward your complaint to that agency.
